The new and improved duplicated pad contains 100 duplicated sheets and has been designed to help reduce customer contact areas.

On completion of the form, customers will take away the white copy, which details information about the piercing and after care.  The green copy is kept by the store.

An integral part of the piercing process to record information, check suitability for the piercing and to provide your customer with the correct After Care procedure to look after their new piercing.
